CD38 is a type II transmembrane glycoprotein with a molecular weight of 46kDa. In 1980, CD38 as a specific marker of T cell activation was discovered by E.L.Reinherz and S.F.Schlossman etc. The overall structure of CD38 is a short cytoplasmic tail at the N-terminus, a single-pass transmembrane domain and a longer extracellular region at the C-terminus. CD38 has cyclase and hydrolase activities and is a bifunctional extracellular enzyme involved in nucleotide metabolism. CD38 uses NAD+ as a substrate to form nucleotide metabolites such as cyclic ADP ribose (cADPR). Nucleotide metabolites are potent second messengers that regulate the mobilization of Ca2+ in the cytoplasm. Then signaling pathways are activated which control various biological processes, such as lymphocyte proliferation, insulin secretion from pancreatic B cells, and T cell activation. CD38 is very important for maintaining the homeostasis of NAD, nicotinamide and other substances. Some studies have also found that CD38, together with CD39, CD73, CD203a, etc., can degrade ATP, NAD+, cADPR, and AMP, thereby producing ADO and inducing an inhibitory immune microenvironment.
CD38 is expressed in a variety of human tissues, including lymphocytes, natural killer cells, T cells, B cells, monocytes/macrophages, pancreas, brain, spleen, and liver cells. The expression of CD38 is associated with a variety of diseases, including AIDS, autoimmune diseases, type 2 diabetes, osteoporosis, and cancer. In recent years, a variety of novel immunotherapeutic approaches targeting CD38 have been validated in pre-clinical models and clinical trials.
