CD248 (Endosialin), also known as TEM1 or tumor endothelial marker 1, is a transmembrane glycoprotein belonging to the C-type lectin domain family. Primarily expressed on activated fibroblasts, pericytes, and tumor vasculature, it interacts with extracellular matrix components like fibronectin and collagen to modulate cell-matrix adhesion and mechanotransduction. CD248 participates in tissue remodeling through PDGF and TGF-β signaling pathways, influencing angiogenesis, stromal activation, and tumor progression. Its overexpression correlates with aggressive malignancies, fibrotic disorders (e.g., idiopathic pulmonary fibrosis), and atherosclerotic plaque instability. Therapeutic targeting strategies include monoclonal antibodies (e.g., ontuxizumab) and antibody-drug conjugates currently in phase I/II trials for solid tumors. Recent studies also explore its role in cancer-associated fibroblast reprogramming and immunotherapy resistance mechanisms.
