The Glial cell line-derived neurotrophic factor family receptor alpha-like protein (GFRAL) is the specific receptor for growth differentiation factor 15 (GDF15). It forms a functional complex with RET tyrosine kinase, activating downstream RAS-MAPK and PI3K-AKT pathways upon ligand binding. This axis regulates energy homeostasis by suppressing appetite, reducing food intake, and enhancing energy expenditure, serving as a key metabolic modulator.
Dysregulation of the GDF15-GFRAL pathway is associated with obesity and cachexia. GFRAL agonists are in early clinical development for obesity, leveraging their anorexigenic effects. For cancer-related cachexia, elevated GDF15 drives weight loss; thus, GFRAL antagonists are being investigated to block this signaling, improving patient appetite and body mass.
