Retinol Binding Protein 4 (RBP4) is the carrier of retinol in blood, a 21 kD protein belonging to the lipocalin family and containing a retinol binding site. Under normal circumstances, plasma RBP4 is mainly synthesized in hepatocytes, followed by adipose tissue, accounting for 15%-30% of its synthesis amount, and can also be secreted by the yolk sac. RBP4 is widely distributed in human blood, cerebrospinal fluid, urine and other body fluids. The primary function of RBP4 is to play an important role in the storage, metabolism, and transport of retinol (also commonly referred to as vitamin A). RBP4 is the most important extracellular transporter, responsible for binding and transporting retinol in the blood. Adipose, kidney, testis, brain and digestive tract tissues can all synthesize and secrete RBP4, but under normal physiological conditions the main source of this protein is the liver.

As a secreted protein, RBP4 has important biological activities in insulin resistance, arteriosclerosis, hypertension, inflammation and other processes. Elevated RBP4 not only causes insulin resistance, lipid metabolism disorder, but also promotes the occurrence and development of arteriosclerosis. These results provide a theoretical basis for detecting serum RBP4 levels as a diagnostic method for cardiovascular-related diseases, and also provide a new idea for our treatment of diabetic macrovascular disease.
