Sjögren's syndrome

Sjögren's syndrome (SS), also known as dry syndrome，is a chronic autoimmune connective tissue disease, mainly triggered by genetic, endocrine, viral infection and other factors, resulting in salivary glands, lacrimal glands and other exocrine glands function is impaired, the clinical manifestations of dry mouth, dry eyes, dry skin, joint pain, digestive and respiatory systems and other multi-systems are affected by the symptoms.

SS can be divided into two types: primary and secondary. Primary desiccation syndrome (pSS) is a condition that is not associated with other autoimmune diseases, while secondary desiccation syndrome is a condition that is comorbid with other autoimmune diseases such as rheumatoid arthritis or systemic lupus erythematosus.

Drug development for SS has focused on the following directions:

• B-cell-targeted therapy: Since SS is a disease characterized by highly active B cells, abnormalities in B-cell subpopulations may play a key role in the development of the disease, and therefore therapeutic strategies targeting B cells have attracted much attention. The main drugs currently under study include rituximab, which targets CD20 on the surface of B cells, Ianalumab, a monoclonal antibody drug that targets BAFF-R, which are all targeted at inhibiting the survival or function of B cells.
• FcRn monoclonal antibody therapy: Nipocalimab is an anti-neonatal Fc receptor (FcRn) monoclonal antibody, which improves the condition of patients with desiccation syndrome by selectively blocking the interaction between IgG and FcRn to reduce the level of circulating IgG including autoantibodies. The effectiveness of this therapy has been demonstrated in clinical phase II studies.
• CD40L antagonists: Dazodalibep is a non-antibody fusion protein that acts as a CD40L antagonist to block costimulatory signaling between T cells, B cells and antigen-presenting cells, thereby reducing levels of a variety of proteins associated with biomarkers of B cells, T cells and dendritic cells in patients with desiccation syndrome.
• TYK2 Inhibitors: Deuterocleboxetinib (BMS-986165), a potent and selective TYK2 variant inhibitor that targets dry syndrome-associated cytokines (including type I IFN, interleukin-12, and interleukin-23) and acts by blocking signaling in these pathways, is approved for marketing in 2023.

Others such as CD19, CD22, IL2R and IL17R are also important targets for drug development in SS. Drugs against these targets are also in clinical development and have shown some efficacy.
