Colorectal Cancer

Colorectal cancer is a malignant tumor originating from the epithelial cells of the mucosa of the colon or rectum, and its incidence remains high worldwide, making it one of the leading causes of cancer-related deaths. Depending on the location of the tumor, colorectal cancer can be classified into colon and rectal cancer. Colon cancer usually originates from polyps in the colon, while rectal cancer tends to occur in the lower part of the rectum. Symptoms of colorectal cancer include blood in the stool, changes in bowel habits, abdominal pain, and weight loss.

The treatment of colorectal cancer has evolved from traditional chemotherapeutic agents to targeted therapies and immunotherapies against specific molecular targets. Below are some important drug targets and their related drug development progress:

EGFR (Epidermal Growth Factor Receptor): EGFR is overexpressed in about 10% of colorectal cancers, and monoclonal antibodies targeting EGFR, such as cetuximab, have been used in the treatment of certain types of colorectal cancer.
RAS (KRAS and NRAS): RAS gene mutations are more common in colorectal cancer, especially KRAS mutations. In recent years, breakthroughs in inhibitors targeting the KRAS G12C mutation, such as Sotorasib (AMG510), have provided new treatment options for patients harboring this mutation.
BRAF: The BRAF V600E mutation is another important therapeutic target in colorectal cancer. The combination of BRAF inhibitors such as Vemurafenib and MEK inhibitors such as Cobimetinib has shown better efficacy in the treatment of advanced colorectal cancers with BRAF mutations2. .
HER2 (Human Epidermal Growth Factor Receptor 2): HER2 overexpression occurs in a subset of colorectal cancer patients, and targeted therapies against HER2 such as Tucatinib in combination with Trastuzumab have been approved by the FDA for the treatment of patients with HER2-positive advanced colorectal cancer.
MSI (microsatellite instability): colorectal cancers with MSI-H or dMMR (mismatch repair defective) respond better to immunotherapy. Immune checkpoint inhibitors such as Pembrolizumab have shown significant efficacy in such patients.
VEGF (vascular endothelial growth factor): Anti-VEGF monoclonal antibodies such as Bevacizumab have been widely used in the treatment of advanced colorectal cancer by inhibiting tumor angiogenesis.
