Immunological Diseases

The immune system is an important system for the body to defend itself against foreign pathogens and maintain internal stability. An immune disorder is a state in which the body's immune system is dysfunctional. This dysfunction may be caused by a variety of factors, including genetics, environment, and lifestyle, and may lead to a variety of diseases. Immune disorders can be divided into two main categories: immunodeficiencies and autoimmune diseases.

Immunodeficiency: Immunodeficiency refers to a low-functioning immune system that is unable to effectively defend itself against foreign pathogens. Immunodeficiency may be congenital, such as congenital immunodeficiency disease; or acquired, such as AIDS and long-term use of immunosuppressants. Immunodeficiency can lead to problems such as susceptibility to infections and slow recovery from illness.
Autoimmune Diseases: Autoimmune Diseases are diseases in which the immune system mistakenly attacks its own tissues, resulting in tissue damage and dysfunction. Common autoimmune diseases include rheumatoid arthritis, systemic lupus erythematosus, and thyroid disorders.

Immunodeficiencies and autoimmune diseases are two important groups of immune-related diseases that have seen significant progress in drug development in recent years:

Drug Development for Immunodeficiencies: Leniolisib, an oral selective phosphatidylinositol 3-kinase δ (PI3Kδ) inhibitor, has been accepted for priority review by the U.S. Food and Drug Administration (FDA) for the treatment of activated inositol phospholipid 3-kinase δ syndrome (APDS), a rare primary immunodeficiency disease. This demonstrates that precision therapeutic strategies for specific immunodeficiency diseases are becoming available.
Drug Development for Autoimmune Diseases: Therapeutic agents for autoimmune diseases include traditional synthetic anti-rheumatic drugs (DMARDs) such as hormones, immunosuppressants, immunomodulators and anti-inflammatory drugs. In recent years, significant progress has been made in the development of biologics and targeted DMARDs, such as large molecule monoclonal antibodies (e.g., infliximab, tolizumab) and JAK inhibitors (e.g., tofacitinib, baricitinib).

In addition, cytokine analogs have an important role in the treatment of immunodeficiency and autoimmune diseases by modulating the immune response. Examples include NKTR-214, a drug that increases the half-life of IL-2 through PEGylation, and drugs that increase cytokine targeting specificity through fusion technology, such as ALT-803 (IL-15 "super agonist" complex). Antibody fusion cytokines, such as IL-2, TNF, IL-15, etc., are targeted for delivery to the site of disease through specific antibody-directed technologies to improve therapeutic efficacy and minimize side effects.
