Nutritional Metabolic Diseases

Nutritional metabolic disorders are a group of diseases caused by abnormalities in nutrient metabolism, including but not limited to diabetes, obesity, cardiovascular disease, and fatty liver. Nutritional metabolic diseases are usually associated with an imbalance in the metabolism of nutrients in the body, and may be caused by a variety of factors such as enzyme defects, abnormal hormone levels, genetic factors, or lifestyle.

Drug development for nutrient metabolic diseases focuses on several key targets, and the following are some of the important targets and their R&D progress:

GLP-1R/GIPR: GLP-1 receptors and GIP receptors are important targets for the treatment of obesity and diabetes. Drugs based on these targets can improve insulin secretion and lower blood glucose levels. For example, Amgen's AMG 133 (Anti-GIPR monoclonal antibody fused with GLP-1) demonstrated significant weight loss in Phase I clinical data for the treatment of obesity.
INHBE: INHBE (Inhibin beta E chain) is a member of the TGF-beta superfamily that is expressed primarily in the liver and is associated with body fat distribution and metabolic health. Loss-of-function mutations in the INHBE gene have been found to be associated with lower waist-to-hip ratios and favorable metabolic profiles, suggesting that INHBE is a potential therapeutic target.
lysosomal AMPK (PEN2/Aldolase): AMPK is a key controller of metabolic homeostasis. By specifically inhibiting PEN2 or Aldolase, cellular glucose starvation can be mimicked and lysosomal AMPK can be activated, thus reducing fatty liver and alleviating hyperglycemia, providing new ideas for metabolic disease treatment.
CIDEB: CIDEB gene is associated with hepatic lipid metabolism, Alnylam and Regenerative Element are collaborating to develop RNAi therapeutics targeting CIDEB, which is expected to enter the clinical development stage, providing a new target for NASH treatment.
SLC transporter protein family: SLC transporter proteins play an important role in a variety of metabolic diseases, including type 2 diabetes, hypertension, etc. Studies of SLC transporter proteins as drug targets have shown that they can transport nutrients, metabolites as well as drug molecules, making them important drug targets for chronic diseases.
PCSK9: PCSK9 (proprotein convertase subtilisin type 9) is a popular target for cholesterol-lowering therapy. PCSK9 inhibitors based on RNA interference technology, such as inclisiran, have been approved for the treatment of hypercholesterolemia.
GDF15: Growth differentiation factor 15 (GDF15) is a signaling molecule that regulates energy metabolism, and its secretion is regulated by hepatic Glce, which affects the homeostatic balance of hepatic lipid metabolism in the body, and it may be a potential target in the fight against obesity1.
SLC13A5: As a target with great potential for development, it is important to study the mechanism of action of SLC13A5 in metabolic diseases as well as drug design and development.
